Payroll Deduction
If you are an employee of UL Lafayette, you can take advantage of UL Lafayette Payroll Deduction for your contribution to KRVS 88.7 Public Radio. In order to have your payment deducted from your UL Lafayette paycheck, you must provide KRVS with a signed authorization form. You may print out the required Payroll Deduction Form and mail it to:
